Churchill Driver Requirements

 

  • CDL with a Z endorsement.
  • One Year Verifiable “OTR” Experience.
  • FAST Card a Plus.
  • Social Insurance Card.
  • Current CVOR / 3 Year Abstract.
  • Birth Certificate, Passport or Permanent Resident Card.
  • 23 years of age or older.
  • Stable Work History.

 

Churchill Contractor Requirements

 

  • Master Business License.
  • Direct Deposit Required, Must be in Name of Your Company.
  • Tractor Must Pass our Inspection
  • Tandem Axle Tractor & Sliding 5th Wheel.
  • Bobtail & Physical Damage Insurance Required.
  • $1,000.00 Escrow – Settlement Deduction @ $50.00 Per Week.
  • WSIB Required, If Employing a Driver.
  • Bill of Sale for Truck.
  • Truck Title / Proof of Ownership .
  • Paid Current 2290 (Heavy Highway Use Tax Form).
  • Current Annual Inspection Form.
  • Tax ID Number (EIN)
