Churchill Driver Requirements:

 ·        CDL with a Z endorsement.

 ·        FAST Card Required.

 ·        Social Insurance Card.

 ·        Current CVOR / 3 Year Abstract and Criminal Search.

 ·        Birth Certificate, Passport or Permanent Resident Card.

 ·        One Year Verifiable OTR Experience.

 ·        23 years of age or older.

 ·        Stable Work History.

 Churchill Contractor Requirements:

 ·        Master Business License.

 ·        Direct Deposit Required, Must be in Name of Your Company.

 ·        Tractor Less Than 6 Years Old.

 ·        Tandem Axle Tractor w/ Sleeper & Sliding 5^th Wheel.

 ·        Bobtail & Physical Damage Insurance Required.

 ·        $1,000.00 Escrow – Settlement Deduction @ $50.00 Per Week.

 ·        WSIB Required, If Employing a Driver.

 ·        Bill of Sale for Truck.

 ·        Truck Title.

 ·        Paid Current 2290 (Heavy Highway Use Tax Form).

 ·        Current Annual Inspection Form.
