Churchill Requirements

 

  • CDL-A with One Year Verifiable “OTR” Experience
  • Social Security Card.
  • Haz-Mat, FAST Card, Enhanced Liccense any or all a Plus.
  • Birth Certificate, Passport or Resident Alien Card
  • At Least 23 Years Old
  • Good MVR and Stable Work History

 

Churchill Contractor Requirements

 

  • Must Have a Registered Business Name (D/B/A) “Doing Business As” or Your Business Must be Incorporated.
  • Bank Account Must be Your Company’s Name.
  • Tractor Must Pass Churchill Inspection
  • Tandem Axle Tractor with Sliding 5th Wheel.
  • Bobtail  & Physical Damage Insurance Required.
  • $1000.00 Escrow Settlement Deduction @ $50.00 Per Week
  • Workman’s Comp Insurance Required if Employing a Driver
  • Bill of Sale for Truck.
  • Current Paid 2290 (Heavy Highway Use Tax Form).
  • Truck Title
  • Current Annual Inspection Form
  • Tax ID Number (EIN).
