Churchill Requirements:

·        CDL-A with HazMat

·        Social Security Card. 

·        FAST Card Approved.                                   

·        Birth Certificate, Passport or Resident Alien Card

·        One year Verifiable OTR Experience

·        At Least 23 Years Old

·        Good MVR and Stable Work History

Churchill Contractor Requirements:

•  Must Have a Registered Business Name (D/B/A) “Doing Business As” or Your Business Must be Incorporated.

• Bank Account Must be Your Company’s Name.

• Tractor Less Than 6 Years Old.

• Tandem Axle Tractor with Sleeper & Sliding 5^th Wheel. 

• Bobtail  & Physical Damage Insurance Required.

• $1000.00 Escrow Settlement Deduction @ $50.00 Per Week

• Workman’s Comp Insurance or Occupational/Accidental Insurance.

• Bill of Sale for Truck.

• Current Paid 2290 (Heavy Highway Use Tax Form).

• Truck Title

• Current Annual Inspection Form

• Tax ID Number.
